**Applications will not be reviewed until the $15 fee is received.

**All incomplete applications (failure to supply all information on the application, including income information) or applications without the $15 fee will not be reviewed. 

Please complete a Mercy Anchor Community Center application and email to mlockhart@mcwerie.org. Your application may also be faxed to (814) 459-7012 attention: MACC Housing, or dropped off at the Mercy Center for Women, 1039 East 27th Street, between 9:00 AM and 4:00 PM, Monday-Friday.

Applicants are also required to provide the following for all household members:

• Birth Certificates
• Social Security Cards
• Photo IDs for everyone 18 years and older
• I-94 Documentation (if applicable)
